Referral Requirements
Written referrals from your GP or Specialist are required for:
Respiratory or Sleep consultations
Diagnostic Sleep Studies
Diagnostic Testing
Lung Function Tests
Your doctor can provide you with the referral form to bring along to your appointment, or they can email or fax it directly to us.
Once received, our physicians will triage your referral, and our team will contact you to arrange a suitable appointment.
GP referrals are valid for 12 months from the first date of service.
Specialist referrals are valid for 3 months from the first date of service.
Preparing for Your Appointment
Please bring:
A list of your current medications
Any relevant chest X-rays or CT scans
Results of previous breathing tests or sleep studies
PLEASE DOUBLE CHECK PREPERAPATION REQUIRMENTS PRIOR TO TESTING
